Sam Colton Hall, una mansión de Liberty Falls, en Virginia Occidental, el 28 de febrero de 1991. Construida por la secta de Josiah Shem sobre el Serpent Mound a partir de 1770 y convertida después en un centro de investigación del Project Janus que albergaba a S.A.M., es adonde Weaver, Grey, Carver y Maya llevan el Sentinel Artifact.

La partida empieza fuera, en los jardines de la mansión junto al Garden Pond, donde la GS45 cuelga de la pared por 750 puntos. Los enemigos son zombis, Armored, Heavy y Toxic Zombies, Elder Disciples, Vermin, Parasites, Manglers, Doppelghasts, Amalgams y Abominations.
