Un centre de recherche secret de l'Atlas Corporation aux États-Unis, en novembre 2054. Atlas a largué son arme biologique Manticore sur une émeute dans une prison de New Baghdad, les morts sont revenus, et certains ont été amenés dans un sous-niveau caché de ce laboratoire pour y être étudiés. Ils s'échappent, et quatre employés restent pour tenir la ligne : Oz le concierge, Lilith la spécialiste informatique, Decker l'agent de sécurité et Kahn le cadre, joués par John Malkovich, Rose McGowan, Jon Bernthal et Bill Paxton. Outbreak est la première map Exo Zombies, réalisée par Raven Software avec Sledgehammer Games, et elle est arrivée avec Havoc, le premier pack DLC, le 27 janvier 2015 sur Xbox. Ses zombies portent des exosquelettes, et avec eux viennent les EMZ qui coupent l'Exo Suit d'un joueur, les Chargers, les Exploders, les Hosts qui infectent et les Security Dogs.

Une partie commence dans la Courtyard, à l'extérieur du complexe, autour d'un bus garé au milieu de la cour, avec un Atlas 45 et quatre grenades à fragmentation. Le MK14 et le Bulldog sont aux murs pour 500 points chacun. Un interrupteur de courant derrière le point d'apparition active Exo Medic, près de la porte de l'Administration : 500 points en solo, 1 500 en coop, et la seule Exo Upgrade qui ne nécessite pas de combinaison. Deux portes à 750 points mènent à l'Administration et au Main Hub, et les Exo Suits sont plus loin, dans l'Exo Testing Room. Les zombies descendent de trois points sur les toits, passent par trois fenêtres et sortent de sous le bus ; Exo Zombies n'a aucune barricade à reconstruire. Les Security Dogs et les Hosts ont leurs propres manches, et un joueur infecté par un Host est guéri quand le dernier Host de la manche meurt, puisque la Decontamination Zone est derrière la porte de l'Administration.

The Outbreak First Room leaderboard on Zombie First Room Rankings tracks every verified run held in the starting room of Outbreak, on Call of Duty : Advanced Warfare. 37 records are listed here, and the best rounds on record are 40 in solo by Carloskrillex, 49 in 2 joueurs by Carloskrillex, 27 in 3 joueurs by MarinaccioFran98 and 16 in 4 joueurs by Skrine.

Outbreak is tracked outside the rankings: its records earn no ranking points, because no ranking takes it. It does carry a difficulty coefficient — solo 6 and two players 5.2 — which is what weighs its rounds in the ZFRR rating of the players who hold them.

First Room is the purest discipline in Call of Duty Zombies: no Pack-a-Punch run around the map, no easter egg, no train through open ground. You hold the spawn room of Outbreak, you keep the doors closed, and you push the round counter as far as the horde allows. Every high round, world record and personal best on this page is backed by its video and checked against the reference boards before it is listed.
