A secret Atlas Corporation research facility in the United States, in November 2054. Atlas dropped its Manticore bioweapon on a prison riot in New Baghdad, the dead came back, and some of them were brought to a hidden sublevel of this lab to be studied. They break out, and four employees are left to hold the line: Oz the janitor, Lilith the IT specialist, Decker the security guard and Kahn the executive, played by John Malkovich, Rose McGowan, Jon Bernthal and Bill Paxton. Outbreak is the first Exo Zombies map, made by Raven Software with Sledgehammer Games, and it came with Havoc, the first DLC pack, on January 27, 2015 on Xbox. Its zombies wear exoskeletons, and the EMZs that shut down a player's Exo Suit, the Chargers, the Exploders, the infecting Hosts and the Security Dogs come with them.

A game starts in the Courtyard, outside the facility, around a bus parked in the middle of the yard, with an Atlas 45 and four frag grenades. The MK14 and the Bulldog are on the walls for 500 points each. A power switch behind the spawn turns on Exo Medic, by the door to the Administration: 500 points in solo, 1,500 in co-op, and the only Exo Upgrade that does not need a suit. Two 750-point doors lead to the Administration and the Main Hub, and the Exo Suits are further in, in the Exo Testing Room. Zombies come down from three spots on the rooftops, through three windows and from under the bus; Exo Zombies has no barriers to rebuild. Security Dogs and Hosts have their own rounds, and a player infected by a Host is cured when the last Host of the round dies, since the Decontamination Zone is behind the Administration door.
